Reema Health is a U.S.-based home healthcare company that connects people facing barriers to care with health and social services. As their operations grew, their care teams needed a better way to communicate securely, access centralized patient information, manage tasks, and support patients in the field without relying on disconnected tools. VT Netzwelt designed and developed a mobile-first care platform built around four core modules: Authentication, Feed, Profile, and Members. The Feed module unified real-time communication, while Profile and Members helped organize patient information and make it easier for care teams to act on important details. Auth0 integration added secure authentication and role-based access, helping the platform support strict healthcare data protection needs. The result was a scalable, easy-to-use platform that helped Reema Health improve coordination, reduce workflow friction, and keep patient support more responsive.
